Save Time When Visiting the Office

All patients to Minor Med will need to fill out a form upon their arrival. To help expidite your visit, you can download and fill out this or other forms before you come in.

Confidentiality

We took confidentiality seriously long before HIPAA required it.  We have annual training sessions specifically on the regulations but we talk about the importance of privacy all the time.  We have a meeting of all the staff quarterly and privacy is always on the agenda.  Each person must sign a document each year which makes them aware of their duties and promising to abide by our privacy policies.
